Adult Foster Care (AFC)
AFC is a specialized form of residential care designed to provide support and assistance to adults who are unable to live independently due to physical, mental, or emotional challenges.
- Be 16 years of age or older
- Your physician must provide a summary confirming the appropriateness of AFC services
- Require daily assistance with at least one Activity of Daily Living (ADL), such as bathing, dressing, eating, continence care, ambulation, or transfer
- MassHealth Itss must approve your clinical eligibility
- Be eligible for MassHealth (Standard, SCO, or be able to pay privately)
- Have a medical diagnosis and need daily personal care
- Be able to communicate his/her needs (it's okay to be confused or forgetful)
- Not be harmful to self or others
- Be compliant with plan of care
Group Adult Foster Care
The
VCare Group Adult Foster Care Program (GAFC) is a Personal Care Assistance Homecare Services Program created to help residents remain in their own homes rather than moving to institutional care. Our team provides compassionate daily support designed to promote independence, dignity, and comfort.

GAFC Program Eligibility
To qualify for the program, participants must meet all of the following criteria:
- 22 years of age or older
- Insured by MassHealth Standard or other specified Senior Care Options and One Care Plans
- Approved by their physician for personal care services
- Reside in “Clustered Housing” (subsidized apartment building)
- In need of daily assistance with personal care
